CERTIFICATE OF DISCIPLINARY HISTORY

A Discipline history certificate provides a certified history of any disciplinary action taken against an attorney since licensure in Colorado. Both public and private information is provided. For confidentiality reasons, only an attorney, upon verification of identity, can request their own discipline history certificate. Requests for another individual’s record will not be honored. Attorneys may request that their discipline history certificate be sent directly to another jurisdiction.

Duplicate Document Requests - If you would like to order multiple documents, a separate request form must be completed for each copy of a document.
